Mersey Care NHS Foundation Trust provides specialist inpatient and community mental health, learning disabilities, addiction services and acquired brain injury services for the people of Liverpool, Sefton and Kirkby, Merseyside. We also provide community physical health services in Liverpool and Sefton, secure mental health services for the North West of England, the West Midlands and Wales and specialist learning disability services across Lancashire, Greater Manchester, Cheshire and Merseyside. Our services are currently provided from 124 sites across the North West.

An exciting opportunity has become available to join are existing Physical Health team. This is a dual role, will delivery training around identified physical health topics, as well providing  physical health clinical cover in the community services and  H&W clinics and therefore requires a clinical member of the team.

You will be situated in the local division within Mersey Care and work towards reducing the premature death of clients with SMI by providing screening and intervening around their physical health needs. The role will align with the Divisions Operational Plan and local KPI's.

The successful candidate will become part of a growing physical health team supported by the Trust Values to create parity of esteem between mental health and physical health.

You will support the Modern Matron for physical health in providing the physical health agenda , training will be provided although prior knowledge of health inequalities experienced by this population would be advantageous, especially around diabetes, COPD and long term conditions. 

The position will require travelling between teams in the Local Division and the ability to work autonomously within the needs of the team.

The new team member will be welcomed into a forward thinking team with integration high on the agenda.
