Mersey Care NHS Foundation Trust provides specialist inpatient and community mental health, learning disabilities, addiction services and acquired brain injury services for the people of Liverpool, Sefton and Kirkby, Merseyside. We also provide community physical health services in Liverpool and Sefton, secure mental health services for the North West of England, the West Midlands and Wales and specialist learning disability services across Lancashire, Greater Manchester, Cheshire and Merseyside. Our services are currently provided from 124 sites across the North West.

An exciting opportunity has arisen for a 12 month 1 WTE fixed term position as Diabetes Specialist Nurse in our Diabetes Service in South Sefton community Division of Mersey Care NHS FT.

The Diabetes service is an established service, with excellent connections across the community footprint, and a close affiliation to the Acute Trust. The service operates with the support of a Diabetes Consultant who provides medical/clinical support. As a well-established service, the team provide a number of clinics across the Division, as well as domiciliary care, clinical support to colleagues, stakeholder engagement and more recently a significant increase in delivery of Diabetes and You structured patient education.

This position will require excellent clinical knowledge of Diabetes, with demonstrable evidence of academic study at Degree/Masters level, experience of managing a defined caseload. You will be expected to support  team during a very exciting time as we move towards a more integrated bio-social model of health care, with Transformation very high on our agenda.
